JOB
Responsibilities:
Provides telephone or in-person evaluation, information, and assistance for long term care applicants Complete Long Term Services & Supports Questionnaire (LTSSQ) and Support Navigation in the Ohio Benefits Long Term Services and Supports (OBLTSS) data system within timeframes identified in contract with ODA and ODM.
Data enter information into PASSPORT Information Management Systems as required by the OBLTSS program.
Complete data entry into the Chart Assignment Log for individuals requesting an assessment.
Provide information on program options and eligibility and community resources.
Assist callers in identifying and securing appropriate programs, services and care settings utilizing available resource directories.
Facilitate the contacting of resources if a consumer is unable to complete or complete a warm hand-off, if needed.
Provide information, assistance and referrals to individuals who walk-in to the Agency and complete the LTSSQ and Support Navigation in OBLTSS.
Utilize Ohio Benefits and Ohio MITS systems to determine individual's Medicaid and managed care status.
Participate in community groups as requested.
Obtain AIRS certification.
Must obtain passing score within three attempts.
Complete Phone Log and Time Card daily to accurately reflect time spent on each activity.
Refer callers to telephonic consumer satisfaction survey per current protocol.
Other Duties Contact community agencies and medical offices to promote agency programs and obtain referrals.
Complete annual Compliance training and other Agency trainings as requested.
Identify and implement LEAN process engineering activities.
Participate in the development and implementation of the local performance culture.
Exemplify the Agency 3C's corporate culture in all activities and interactions with staff, members, consumers, stakeholders as well as the community at large.
Maintain minimum proficiency in all requisite computer applications and software.
Participate in the implementation of strategic business and/ or operational plans, projects, programs and systems as appropriate for the objectives.
Participate in and contribute towards accomplishment of divisional and Agency dashboard goals.
Perform other tasks as assigned by supervisor.
Position Qualifications Required:
1 to 3 years in a customer service, social service or medical setting High School diploma Preferred:
Above average accurate typing skills Prior experience with conflict resolution or customer service Experience with Microsoft Office products Bachelor's degree in a related field Prior experience in a nonprofit setting Prior experience with Medicare and/or Medicaid services Prior experience with aging and disabled population Prior experience with long-term care services, Medicaid waiver and/or mental health services Upon hire:
Completion of OBLTSS training and PASSPORT Modules within first 90 days of employment.
AIRS certification when eligible for testing Office based position with possibility to participate in remote work pending successful orientation Physical/Mental Position Qualifications This position requires its occupant to have the following abilities to successfully perform the essential functions of the job:
Ability to effectively communicate with the elderly population and general public, both in person and by phone, Ability to comprehend written material related to essential job functions, Ability to accurately document facts related to essential job functions, Ability to operate a computer well enough to perform essential job functions, Ability to comprehend, recall, and apply facts related to essential job functions, and Ability to analyze, evaluate and implement a reasonable course of action based on available information.
Proof of influenza vaccination is required.

The check will bounce and the victim is left responsible.
