Patient Care Coordinator
Visiting Physicians Association, a national leader in delivering home based primary care, works with home health and hospice agencies to ensure patients receive continuity of care in the home environment. VPA in-home physicians collaborate with agencies to establish in-home treatment plans and certify the need for services. VPA partners with over 1,000 home health agencies, collaborates with independent and assisted living communities, and works with skilled nursing facilities and hospitals nationally to coordinate services and patient transitions to home care.
POSITION DESCRIPTION
The Patient Care Coordinator (from here on designated as PCC) is a very unique position. The PCC knows protocol, knows the limits and regulations of the position, operates within their scope of practice, and focuses entirely on aiding the Provider in taking care of his/her patients.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Responsible for answering calls and collecting all pertinent medical information needed for the providers medical decision making
Responsible for processing Provider orders
Responsible as the point person for the Providers patients, facilities, and other healthcare professionals
Scheduling consult and specialist appointments
Requesting medical records
Documenting all calls in EMR
The ability to perform continuous detailed tasks with frequent interruptions
Collaborating with other divisions within Care Center
Reviewing and assigning faxes and attachments
Working with Care Center RN when triage is needed and urgency is escalated
Ability to work as a team to cover other PCCs without being asked
Ability to develop strong rapport over the phone
Maintains patient confidentiality and all HIPPA privacy requirements
Attends required meetings and in-services
Ability to adapt and apply guidelines and procedures
Strong customer service approach to all callers
Performs related work as required
PHYSICAL REQUIREMENTS/WORKING CONDITIONS
Full range body motion including handling/lifting patients
Ability to drive up to 150 miles daily in urban and/or rural settings
Ability to lift/carry items weighing up to 50 lbs
Ability to read and understand road maps
Sit or stand for long periods of time
REQUIRED KNOWLEDGE, SKILLS, AND EXPERIENCE
Certified (AAMA) MA, LPN/LVN, EMT, Military Medic, Paramedic, Foreign Nursing Grad, or Foreign Medical Grad
Washington Practices Only Must be an LPN/LVN
Minimum 2 years office experience in health care coordination
Strong medical office call experience with ability to collect appropriate medical information from the patient/caller to communicate to provider
Basic working knowledge of medical terminology
Computer skills: Microsoft Excel, Word, Outlook
Strong customer service, organizational and interpersonal skills with excellent multi-tasking and prioritization
Professional demeanor, self-motivated, and dependable
Ability to communicate effectively with all personnel levels within the organization
Ability to work closely with Patient Care Center RN and Provider
Strong phone communication skills
PREFERRED KNOWLEDGE, SKILLS, AND EXPERIENCE
Experience in EMR strongly preferred

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
